In addition, we work closely with our valued partners including physicians, physical and occupational therapists, attorneys, continuing care retirement communities (CCRC’s), assisted living and rehabilitation facilities, hospitals and nursing homes, local and federal governmental agencies on aging, and other referral sources and experts to aid our clients and families connect with the help and resources they need.


DAILY OR WEEKLY SERVICES MAY INCLUDE:

SUPPORTIVE SERVICES:

  • Telemedicine and internet support
  • Respite Care relief support for family, friends, and caregivers*
  • Regular communication between care providers, clients, families and 3rd-parties
  • Daily and as needed wellness check-in when clients family is out of town*
  • Transitional Care Support Services*
  • Geriatric Care Management Services*

NON-MEDICAL SERVICES MAY INCLUDE:

  • Hourly to live-in 24 hours a day, 7 days a week
  • Short and long term assistance
  • Blood pressure, fluid monitoring and charting, and other vitals monitoring and checks
  • Ambulation (walking) and transfer assistance
  • Companionship & socialization such as reading, taking walks, conversation, and assistance with arts and crafts
  • One-on-one care and monitoring to help with fall prevention (and for our cognitively challenged clients who may wander)
  • Assistance with Activities of Daily Living (ADLS) such as bathing, toileting, dressing & grooming support and assistance
  • Assistance with light exercise and wellness techniques as prescribed by clients physician, physical and occupational therapists
  • Meal planning, preparation and cooking
  • Encourage walks (or in a wheelchair) on a nice day (weather permitting and as tolerated)
  • Medication reminders provided by a C/GNA
  • Works with third party providers including physicians, nurses, social workers, and palliative care and hospice partners to deliver safe and effective care

SKILLED SERVICES MAY INCLUDE:

  • Hourly to live-in 24 hours a day, 7 days a week
  • Short and long term assistance
  • Medication Dosage & Administration (LPN/RN/NP)
  • Wound Care, G-Tube, Colostomy Care, etc. (RN/NP)
  • Works closely with 3rd-party providers including physicians, nurses, social workers, and palliative care and hospice partners to deliver safe and effective care (LPN/RN/NP)
  • Manages orders by doctors and delivery of best care practices (RN/NP)
  • Establishes care plan for clients individual care needs (RN/NP)
  • Coordinates and communicates with care team, clients and families (LPN,RN,NP/LCSW)
  • Provides transitional care for clients discharging from hospital, rehabilitation facilities, nursing homes, etc. (RN/NP/LCSW)
  • Provides care management as needed and desired by clients (RN/NP/LCSW)
  • Orders prescriptions and supplies for medically necessary skilled services as ordered by clients physician (LPN/RN/NP)
  • Other skilled services may be provided!

FORMS OF PAYMENT:

  • Private Pay
  • Assistance with Long-Term Care (LTC) reimbursement
  • We do NOT participate with Medicare, Medicaid or private health insurance!
