At Hearts of Ireland our goal is to help our Client's find and select the most qualified care providers, including Registered Nurses (RN), Licensed Practical Nurses (LPN), Certified and Geriatric Nurse Assistants (C/GNA) who can offer the helping hands and assistance needed for those facing recovery, chronic or advancing illness, and behavioral health needs. To help facilitate these increasingly complex levels of care we offer Care Coordination services that can help maximize outcomes. As clients and families approach some of these tough decisions it can be comforting to know our entire team will be here every step of the way ensuring peace of mind, improved outcomes, and a smoother transition for all.
Care Coordination services assists with:

  • Transitions, Hospital/Doctor-Patient-Family relations, Medication oversight and compliance, improved Self-Management, Prevention, and Care Continuum

  • Coordinating Short and Long-Term services
  • Provided by CRNP

  • Customized packages based on Client's needs (Fee for Service)

Daily or weekly services include: 

  • Hourly to Live-In (24 hours a day, 7 days a wk; Based on Provider availability)
  • Long and Short-term assistance
  • Assistance with Activities of Daily Living (ADLs) such as bathing, grooming, toileting and feeding assistance provided by a certified and experienced Nurse Assistant
  • Companion services (provided by a C/GNA) such as reading, taking walks, conversation and assistance with arts and crafts
  • Respite Care (relief support for family, friends and Caregivers)
  • Assistance with ROM (Range of Motion) and Physical Therapy exercises
  • Light housekeeping and laundry (wash, dry, fold/iron and put away)
  • Meal planning, preparation and cooking
  • Transportation and Errands such as shopping, doctor visits, visiting family and friends, going to see a movie and going out to dinner (service is at the discretion of the independent contractor and client must be ambulatory with no history of falls within the past 6 months)
  • Medication reminders provided by a C/GNA
  • Medication Dosage & Administration provided by a RN/LPN
  • Wound Care, G-Tube, Colostomy Care, etc provided by an RN
  • Assistance with scheduling appointments and calling in/picking up orders
  • Assistance with medical equipment and prescription deliveries
  • Non-medical & Skilled Care

  • Assistance with Long-Term Care (LTC), VA Benefits, Private, and Life Insurance Reimbursement
