At Hearts of Ireland our goal is to help our Clients' find and select the most qualified care providers, including Social Workers (LCSW), Registered Nurses (RN), Licensed Practical Nurses (LPN), Certified and Geriatric Nurse Assistants (C/GNA), who have a deep understanding of their needs, and can offer the helping hands needed, which can be highly beneficial to those Clients' facing recovery, chronic or advancing illness, and mental health needs. To help facilitate these increasingly complex levels of care, we can offer Care Coordination services to help maximize outcomes. 
How does Care Coordination work? 
Upon starting services with our agency, we can arrange for an Registered Nurse (RN) to visit with you, and your family/POA/Advocate, to complete a Skilled Nursing Assessment. This information will be assessed by a RN and Social Worker (LCSW) affiliated with our agency. Upon completion, a LCSW will arrange a 2nd visit to your home, and go over the assessment with you and your family, including exploring all options as you navigate the process, and plan for in home care or alternative living arrangements specific to you or your loved ones needs. Understandably, our Clients' prefer the familiarity, and comforts of living at home, so we strive each and everyday to make this possible. In some cases, we understand living at home may no longer be realistic or a safe option. When this happens, it can be a very stressful time for our Clients' and their families. By offering the support families need as they approach some of these tough decisions, it can be comforting to know our entire team and contractors will be here every step of the way- ensuring peace of mind, improved outcomes, and a smoother transition for all involved.
Daily or weekly services include: 

  • CARE COORDINATION (provided by RN/LCSW) assists Clients' with: 

     * Transitions, Doctor-Patient-Family relations, Medication oversight and
        compliance, improved Self-Management, and Care Continuum 

     * Coordinating Short and Long-Term services

     * Co-morbidness, Mental Health related needs, and Acute or Early Onset aliments

     **Care Coordination offers customized packages to Clients' (Fee for Service)

  • 24/7 availability and emergency coverage 
  • Hourly and 24 hour coverage (Min of 4 hours per shift)
  • Long and Short-term assistance 
  • Assistance with Activities of Daily Living (ADLs) such as bathing, grooming, toileting and feeding assistance provided by a certified and experienced Nurse Assistant
  • Companion services (provided by a C/GNA) such as reading, taking walks, conversation and assistance with arts and crafts
  • Respite Care (relief support for family, friends and Caregivers)
  • Assistance with ROM (Range of Motion) and Physical Therapy exercises
  • Light housekeeping and laundry (wash, dry, fold/iron and put away)
  • Meal planning, preparation and cooking
  • Transportation and Errands such as shopping, doctor visits, visiting family and friends, going to see a movie and going out to dinner (service is at the discretion of the independent contractor and client must be ambulatory with no history of falls within the past 6 months)
  • Medication reminders provided by a C/GNA
  • Medication Dosage & Administration provided by a RN/LPN
  • Wound Care, G-Tube, Colostomy Care, etc provided by an RN
  • Assistance with scheduling appointments and calling in/picking up orders
  • Assistance with medical equipment and prescription deliveries
  • Non-medical services provided by a C/GNA
  • Hands on assistance services provided by a RN/LPN (Skilled Care)

  • Assistance with Long-Term Care (LTC) Reimbursement
