HEARTS OF IRELAND Referral Service Agency of MD, LLC is a premier nurse referral service agency providing concierge support services including non-medical and skilled care, assisted living placement, transitional care, and geriatric care management to clients in the privacy and comfort of their homes. We also offer support services and referrals to other local agencies, hospitals, and facilities, while also helping families connect with other valued resources and our trusted care partners throughout Maryland.
We offer Hourly and Live-in Home Care Nurse Referral Services to support our clients in whatever place they call home. Whether it’s the family home they've shared so many wonderful memories throughout the years, a condo, a retirement living apartment, an assisted living apartment, or even a rehabilitation facility and hospital! We provide the balance seniors and families need to help relieve worries and pressures relating to the health and welfare of our clients and their environment. We are devoted to helping our clients age in place in the comforts of home, or our clients new home as each of our clients needs may vary.
Our passion for helping those in need stems from almost 30 years of direct care experience by our professionally trained and licensed caregivers and nurses, not just from caring for our valued clients, but our own family as well.
